WHAT ARE “PROOF OF PARTICIPATION” DOCUMENTS?

Proof of Eligibility documents are copies of your cards or letters that show you are currently participating in a Government program. Receiving your Proof of Eligibility documents is necessary for American Assistance™ to approve your application and ship out your smartphone. By sending your Proof of Eligibility documents, we are able to verify your eligibility and your identity.

WHAT “PROOF OF PARTICIPATION” DOCUMENTS DO I NEED TO UPLOAD?

To get your application approved and your phone shipped right away, you need to upload your proof documents. Send us EITHER of the items below:

A copy of Proof of Participation in a Government Program approved in your state

-OR-

A copy of Proof of Income

NOTE: we only need ONE proof of eligibility. If you participate in a government program AND you have low income, you do not need to upload both proof copies. Just choose one. Also, if you participate in more than one government program, just upload a proof copy of ONE. The system will request you to upload a copy of your ID as well.

WHAT TYPES OF PROOF OF PARTICIPATION IN GOVERNMENT PROGRAMS DO YOU ACCEPT?

To qualify for American Assistance Lifeline please submit a Proof of Eligibility document showing you participate in a qualifying program. Here are the most common types of proof documents:

Supplemental Security Income (SSI)

  • Copy of a Benefit Check Stub from the Social Security Administration
  • Copy of your Social Security Statement of Benefits
  • Copy of other official documentation from State Agency indicating current participation

Food Stamps, or Supplemental Nutrition Assistance Program (SNAP)

  • Copy of your Award Letter
  • Copy of your Food Stamp Benefit Card
  • Copy of other official documentation from State Agency indicating current participation

Medicaid

  • Copy of your Medicaid ID Card
  • Copy of other official documentation from State Agency indicating current participation

Section 8, or Federal Public Housing Assistance

  • Copy of your Award Letter
  • Copy of your Public Housing Assistance Lease Agreement
  • Copy of your Section 8 Voucher
  • Copy of other official documentation from Public Housing Agency

Veterans Pension or Survivor’s Pension

  • Veterans Pension Grant Letter
  • Veterans Pension COLA Adjustment Letter
  • Survivors Pension Benefit Summary Letter

We accept several types of Proof of Income documentation. Must provide 3 consecutive months, if noted. Here are all of the types we accept:

  • Paycheck Stub-Must provide 3 consecutive months
  • Income Statement from Employer or W2
  • Prior Year’s State or Federal Tax Return
  • Retirement or Pension Statement of Benefits-Must provide 3 consecutive months
  • Social Security Statement of Benefits-Must provide 3 consecutive months
  • Unemployment or Workers’ Comp Statement of Benefits-Must provide 3 consecutive months
  • Veterans Administration (VA) Statement of Benefits-Must provide 3 consecutive months
  • Child Support Award
  • Divorce Decree
  • Federal notice letter of participation in General Assistance-Must provide 3 consecutive months

WHAT TYPES OF “PROOF OF ID” DO YOU ACCEPT?

We accept several types of Proof of ID. Please make sure your ID is valid, not expired and they must be your documents. Make sure that we can read the copy of the ID and that important information hasn’t been cut off. 

Here are all of the types of I.D. we accept:

  • Driver’s License
  • U.S. Government, Military, OR State I.D., which includes Date of Birth and/or Social Security Number
  • Certificate of Naturalization
  • Certificate of US Citizenship
  • US Passport
  • Permanent Resident
  • Permanent Resident Alien Card
  • Military ID
  • Military Discharge which includes Date of Birth and/or Social Security Number
  • Weapons Permit which includes Date of Birth and/or Social Security Number
  • Unemployment / Workers’ Compensation Statement which includes Date of Birth and/or Social Security Number
  • Social Security Card
  • Birth Certificate
  • Statement of Benefits which includes Date of Birth and/or Social Security Number
  • SSA-1099

    NOTE: If you have more than one form of ID, just upload one copy.

HOW DO I GET MY “PROOF” PICTURES READY FOR UPLOAD?

Uploading your proof documents online via desktop or a mobile phone or tablet is quick and easy!

Steps for a Desktop Computer 

Get your Proof of Participation documents. Make sure you take a very clear picture of the document or card and save it to your Desktop file on your computer. Now, get your Proof of ID. Make sure you take a very clear picture of the document or card and save it to your Desktop file as well. 

Once you have filled out your personal information on the application, select the benefits you receive, then click on “Next” at the bottom of the page. Our program will ask you to click on this box and go to your Desktop or Photo Library file where you just saved the 2 pictures. Click on the appropriate Proof of Participation document and that will upload your file to our system. Now, scroll down to the next box and repeat the process for your Proof of ID document. 

After submission, please give us 24 to 48 hours to process and verify your proof documents. Once we’ve reviewed and accepted your proof, we’ll ship out your FREE smartphone right away!

Steps for an Apple or Android Smartphone or Tablet

Get your Proof of Participation documents. Make sure you take a very clear picture of the document or card and save it to your Desktop file on your computer. Now, get your Proof of ID. Make sure you take a very clear picture of the document or card. 

If you are using an Apple Smartphone or Tablet, once you have filled out your personal information on the application, select the benefits you receive, then click on “Next” at the bottom of the page. Then select the “+” sign on the application form and you’ll find the pictures of your Proof of Participation and Proof of ID already in your photo library.

OR

If you are using an Android Smartphone or Tablet, once you have filled out your personal information on the application, select the benefits you receive, then click on “Next” at the bottom of the page. Then select the “+” sign on the application form and then on your phone, you will have the choice to take the photo now or you can upload it from gallery.

After submission, please give us 2 to 3 working days to process and verify your proof documents. Once we’ve reviewed and accepted your proof, we’ll ship out your FREE smartphone right away!

WHAT KIND OF FILE FORMATS ARE ALLOWED FOR UPLOADING MY PROOF DOCUMENTS ONLINE?

You may upload a variety of file formats in our online system. When you upload your proof documents, please make sure the files have one of the following extensions:

  • PDF
  • JPG
  • PNG
  • GIF

    All documents with other file extensions will not be accepted.

HOW DO I KNOW THAT YOU RECEIVED MY PROOF DOCUMENTS?

You will receive an email letting you know that your images were successfully attached.

WHAT IF MY PROOF DOCUMENTS ARE NOT APPROVED?

You will receive an email letting you know that your images weren’t uploaded. You will then need to resubmit your documents to successfully complete your order.
